We are accepting new patients!

In an effort to help reduce the spread of the coronavirus that causes the disease COVID-19, Dr. Jacobs is now accepting new patient consultations by telemedicine.  All consults will be done via our HIPAA compliant video platform Zoom. Video visits allow patients to connect face-to-face in real time without leaving their home. Patients can use a smartphone, tablet or computer.

Step 1: Request Your Appointment

Request an appointment using our online form and a member of our team will contact you within one business day.

Do you have insurance? 

Have you checked to see that we are in-network with your plan?  Dr. Jacobs and Fertility Centers of Illinois (FCI) participates in many health insurance plans.  For a current listing of all plans with FCI is contracted, visit our website

If you are not sure if your health insurance covers fertility services, when you book your initial visit our office will help you determine coverage by contacting your insurance company for a summary of your fertility benefits and will give you a copy of your fertility coverage, so you know what treatments you can pursue when Dr. Jacobs is discussing your treatment options.

No insurance or infertility coverage?

If you do not have medical insurance, you will be registered as a Self-Pay patient. Knowing the cost prior to initiating therapy is important so you know how to plan the financing of treatment. There are several medical factors to discuss in order to achieve a healthy pregnancy. Therefore, it is hard to give exact costs prior to seeing a fertility specialist who can tell us exactly what you need. Once we know what treatment has been recommended by your specialist, only then can the cost be accurately determined.  Our financial counselors can help explain these programs in more detail after you meet with Dr. Jacobs. 

Step 2: Send Your Medical Records

  • Medical records results such as previous fertility treatment, pap smear, hormone testing, semen analysis.

  • From there, your medical team will contact Dr. Jacobs and his team and send your information to them for review.

Step 3: Receive Your Appointment Confirmation

Your patient service coordinator will confirm your appointment once Dr. Jacobs has reviewed your records and has determined the best course of action. We’ll send you an email with the name of Dr. Jacobs, the time and date, and other important information you need to know before the Zoom meeting.
